Thank you for your interest in USIDNET’s FILL Program. USIDNET has been collecting medical information on patients with diagnosed Primary Immunodeficiencies since 1992, and now, as population screening efforts have expanded, so have we expanded our efforts to include babies who are identified as having suspected or possible primary immune deficiencies. We feel that it’s important to collect data on the diagnosis efforts and treatment of these suspected diseases during the first year of life for use in medical research.
If you would like to enroll your child into USIDNET’s FILL Program and allow your child’s medical data to be stored in our database for use in research, please click here for further information and to access our IRB-approved consent form.Patient Self-Consent Form
If you have previously enrolled your child into USIDNET’s FILL Program but have changed your mind and would like to remove your child’s information from our database, please contact us. We will remove the information upon your request.
